Kill Grotesque Plaguebearers in the Chaos Wastes around 48000,53000 until one drops a Pestilent Organ. Click the Pestilent Organ for the unlock.

• Order: Killing these mobs also counts towards the Kill Quest: Daemon Bounty.

This may take some time before one drops the item.
